1 month – Hep B2 month – DTaP, Polio, Hib, Prevnar, Rotavirus

4 month – DTaP, Polio, Hib, Prevnar, Rotavirus

6 month – DTaP, Polio, Hib, Prevnar, Rotavirus

9 month – Hep B

12 month – Varicella, MMR, Prevnar (Hgb & Lead)

15 month – Hib, Hep A

18 month – DTaP

2 year – Hep A (Hgb & Lead)

3 year – none

4 year – DTaP, Polio

5 year – Varicella, MMR (Proquad)

11 year – MCV, Tdap, Recommend HPV series

17-18 year – MCV

Adult Immunization Schedule

Adacel Vaccine (Over age 18) – Every 10 years

Shingles Vaccine (Over 50) – Once in a lifetime

Pneumococcal Vaccine (Over 65) – Every 5 years

Flu Vaccine (Over 18) – Yearly
